PATIENT CARE COORDINATOR
Lighted Pathways Registered Nurses manage all aspects of the hospice patients care, including
coordination of plan of care, pain and symptom management, and family/Caregiver needs, from
admission through day of discharge.  PCC's take the lead role in coordinating the patient's care with
other members of the Interdisciplinary Group, including Physicians, Social Workers and Chaplains.

Lighted Pathways is committed to helping our employees maintain a flexible work schedule in order
to better manage their family and personal lives.  Our PCC's set their own appointments with their
patients.  We have some of the lowest patient to RN case management loads in the hospice
industry.  

Lighted Pathways is looking for currently licensed RN's who have prior independent work
environment experiences such as ICU/CCU, home health, or hospice case management.

Patient Care Coordinator positions are full time salaried positions, maintaining business hours.
PART TIME / PRN / ON-CALL NURSES
Lighted Pathways care team also includes part time and PRN Registered Nurses to support Patient
Care Coordinators in the daily care of hospice patients.  Part Time/PRN RN's provide patient visits,
admissions, on-call and triage nurse duties.  These support nurses must meet all of the same
qualifications of a Patient Care Coordinator, and are usually compensated on a per visit rate.  
Hospice is a 7 days a week, 24 hours a day operation, providing many opportunities for flexible work
schedules.
SOCIAL WORKER
Lighted Pathways Social Workers are an integral part of the Interdisciplinary Group team approach to
hospice patient case management.  Our Social Workers provide for the direction of social services to
our patients, as well as their families, and our staff.   Hospice Social Services includes psycho-social
assessments on all patients, counseling services, and coordinating community resources as
needed.

Lighted Pathways is looking for Social Workers who have completed at least a Bachelors Degree in
Social Work from an accredited university, hold a current state license and have at least 2 years of
verifiable work history in a health care social services position.

Social Work positions are available both full time salaried or part time compensated per visit.
CHAPLAIN / BEREAVEMENT COORDINATOR
Lighted Pathways Chaplains are one of our most important members of the Interdisciplinary Group.  
Chaplains are responsible for coordinating all spiritual concerns of our hospice patients.  Chaplains
support hospice patients and their families, meeting each individual spiritual and emotional need.   
Non-denominational Chaplains are willing to meet each patient's spiritual need regardless of
theology.

Lighted Pathways Chaplains are also responsible to support Caregivers and family members
through the difficult bereavement process, supporting them with follow up visits, phone calls, and
printed materials.

Lighted Pathways Chaplains participate in our bi-annual Memorial Service, honoring all patients who
passed away during the previous 6 month period.

Individuals interested in a Chaplain position must demonstrate a verifiable work history in spiritual
care, preferably in a health care field.  A Bachelor's Degree in a related field is desired.
NURSE AIDES
Hospice Nurse Aides provide personal care for our patients, adding to the quality of daily life in
immeasurable ways.  Lighted Pathways Nurse Aides support patients and family needs in many
ways beyond the activities of daily living.  Our Nurse Aides spend quality time with each patient,
befriending them, comforting them, and providing emotional support along with physical care.

Lighted Pathways employs individuals who are Certified Nurse Aides (C.N.A.) with verifiable past
work experience.  Experience in Nursing Facilities or hospice/home health agencies is preferable.
